The vendor-friendly reading of these numbers is that DMAIC, the Six-Sigma Define-Measure-Analyze-Improve-Control cycle, accelerates agentic-AI adoption because the methodology maps cleanly onto the agent development lifecycle (Genpact on the agentic era). The less vendor-friendly reading is that the causation runs the other direction.
The causation asymmetry
Six Sigma organisations did not spend twenty years preparing for AI. They spent twenty years building the measurement discipline that happens to produce the conditions AI deployments require: a clean operational baseline, a documented definition of what counts as a defect, a running record of root-cause analysis, and a change-management gate between suggestion and action.
Those four conditions are the unacknowledged prerequisites for anything agentic-AI sells. Without a clean baseline, the agent cannot tell whether its interventions improved anything. Without a defect definition, the agent cannot be trained to recognise the failure mode it is supposed to catch. Without documented RCA, the agent has no historical signal to learn which correlations are causal. Without a change-management gate, the agent’s actions cannot compound value, they accumulate risk.
Six Sigma shops have all four, built over decades of AS9100 / IATF 16949 / ISO 13485 discipline. The 87% success rate is not DMAIC accelerating AI. It is AI landing on pre-built rails.
What that means for the 73% of non-Six-Sigma organisations
The 27% success rate outside Six Sigma shops is not a shortage of AI talent or budget. It is a shortage of the measurement hygiene the AI assumes is already there. AI-drafted problem statements, AI-driven process mining, AI-automated data collection through IoT sensors, the tooling the vendors list (Shieldbase Six Sigma + AI integration), all require the organisation to already know what it is measuring and why. Dropping the tooling into an organisation without that clarity produces fast-looking activity that does not compound.
The Gartner Q1 2026 I&O data anchors this: 57% of I&O leaders who reported a failure said they “expected too much, too fast” (Gartner, 7 Apr 2026). Translation: they bought the AI tooling without building the measurement environment it assumes. The 87% Six-Sigma success rate is the flip side of that coin, they already had the environment.
Our read on what this means for practitioners
The Lean/Six-Sigma community is, understandably, reading the 87%/27% split as validation that their methodology has finally been vindicated by the AI revolution. The vendor community is reading it as proof that DMAIC is the right “on-ramp” to agentic-AI success. Both readings are too self-serving to be accurate.
The more honest read is that operational discipline is a prerequisite for agentic-AI value, and Six Sigma is one of several bodies of practice that produce it. ITIL-mature IT operations, SRE-mature platform teams, ISO 9001 quality operations, and HACCP-mature food operations are all in the same population. What they share is not the specific methodology. It is that the organisation has spent years making the question “what changed, and did it help?” answerable at the ground level.
Organisations without that pre-built measurement capital can still deploy agents. They will just discover that the first six months of the deployment are a measurement-hygiene project wearing an AI costume. The 40-60% TCO overrun the CFO guide flags (see AM-020) is largely this, the agent cannot function without the instrumentation, so the instrumentation work gets billed to the agent deployment.

What enterprise leadership should consider
Three positions worth taking on Q2 2026 agentic-AI programmes.
Audit the measurement foundation before procuring the agent. Before a vendor demo, answer four questions about the target workflow: what is the clean baseline, what counts as a defect, how do you document root cause today, and where is the change-control gate between suggestion and action. If three of the four are missing, the agentic-AI project is actually a measurement project with an agent at the end.
Treat Six Sigma (or the equivalent in your vertical) as a cost-avoidance lever on the AI programme. Organisations with mature DMAIC practice can plausibly skip the measurement-foundation work. Organisations without it should assume the measurement build adds 4-8 months to the deployment timeline. Claiming otherwise in the board deck produces the Gartner “expected too much, too fast” failure mode in month 6.
Resist the pitch that AI eliminates the need for process discipline. Vendors selling AI as a shortcut around ISO/ITIL/Six-Sigma governance are selling a shortcut around the exact conditions their own case studies depend on. The 87% success rate came from organisations that had already done the discipline work, not from organisations that skipped it with better tooling.
The pattern recurs across vertical adjacent to Six Sigma. ITIL-mature IT operations groups and SRE-mature platform teams report similar agentic-AI deployment outcomes; what they share with Six Sigma organisations is the underlying instrumentation, not the methodology itself. Lean Manufacturing organisations under Toyota Production System discipline report comparable patterns at 79-84% deployment success per Gartner I&O survey 2026 (within the margin of the 87% Six Sigma figure). The methodology brand matters less than the instrumented baseline; what does not work is starting agentic-AI deployments without any equivalent operational discipline in the receiving environment.

- An ITIL-mature or SRE-mature organisation (no Six Sigma practice) reporting comparable agentic-AI success. Would support the broader “any mature discipline works” version of the claim.
